Thank you for your interest in the Monday Program! To facilitate the student registration process, we have added the forms that need to be completed prior to the Open House at 6:00pm on Friday, August 21st. For the first, or only, student, please download and print the following forms:

The Medication Authorization Form will need to be completed for each student that has medications.
Thank you for expediting your registration process by completing these forms prior to coming to the Open House. If you are unable to download and print the forms, we will provide them at the Open House but it will take you a few minutes to complete them at the end of the orientation.
